Mounted Middleware Bleed-Through

A middleware on a parent FastAPI app is executed for requests that match mounted sub-app routes, so mounted subapi traffic (including docs/openapi) cannot be isolated; fix by scoping middleware to the subapi or conditionally bypassing mount prefixes.
