Update the PDF structure tagging implementation to correctly create and reference marked-content (PDMarkedContentReference with MCID), store structure elements in the ParentTree as a COSArray (even for a single element), set structure tree root parent tree keys correctly, and add the required PDF metadata/viewer/accessibility settings so PAC can resolve the tagged objects.
